· Alert Dr. Portland's nurse you want to proceed with surgery and select a date

· Dr. Portland's nurse will forward your information to billing people within his office who will contact your insurance carrier and get preauthorization

· If you have a work injury, your information will be forwarded to your case manager. They will inform us when your surgery has been approved
