· If you are under 40 years old and have no medical issues, Dr. Portland can perform a history and physical the day of surgery

· If you are under 40 years old and have significant medical history, you need a history and physical performed by your internist or pediatrician within 30 days of surgery

· If you are older than 40 years old you will need a history and physical performed by your internist within 30 days of surgery

· Make sure you have the appropriate requisition for blood work and ekg from Dr. Portland's nurse before your physical
